Led by our empathetic care teams, guided by proven care models, and powered by our own technology, we deliver a level of service that our communities rightfully deserve but rarely receive.Ā \n\n\nWhile our headquarters is in Bethesda, MD, our teams are distributed across the country.Ā  If you’re eager to make a tangible difference in people’s lives, to help correct long-standing disparities in health care, join us.Ā \n\n\nAbout the role:\n\n\nAs a Value Based Care (VBC) Medical Record Specialist for Accompany Health (AH) you will be:\n-Performing external medical record note review to populate pertinent historical information in the patient’s Accompany Health (AH) athenahealth record.\n-Sending external Medical Record Requests and obtaining then categorizing the records when they return.\n-HEDIS measure prep to alert the provider of current results and open gaps that need to be addressed.\n\n\n\n\n\nResponsibilities will include:\n* Review collected medical records housed in a repository, through the athenahealthĀ  Electronic Medical Record (EMR) or via EMR access to outside facility records, for pre-visit chart prep.\n* While reviewing the above medical records, identify key evidence for quality gap closure and transfer that data to designated locations within the patient’s AH athenahealth medical record.\n* Identify and transfer pertinent lab results to populate the AH athenahealth medical record.\n* Summarize identified trends with each patient (i.e., high ED use, frequent hospitalizations), and communicate findings to the clinician via EMR notes.\n* Categorize key documents into appropriate chart note sections for clinicianĀ  ease of review.Ā \n* Successful closure of medical record requests, using follow up to facilities to obtain the requested records.Ā \n* Accurately identify documents and categorize them appropriately in the AH EMR.Ā \n* Using the AH proprietary platform to identify HEDIS measure gaps and closures and complete the designated Quality and Preventive Services template in the EMR to assist the provider in determining open gaps.\n* Other duties as assigned.\n* This role reports to the Lead Medical Record Specialist.\n\n\n\nWhat makes you a fit for the team:\n* You are excited to work in a startup environment, with the ambiguity and shifting priorities that might come with it at times.\n* You are willing to go the extra mile no matter what.\n* You are passionate about our mission to improve the lives and healthcare outcomes of marginalized communities.\n\n\n\nDesired skills and experience:\n* Required\n* Associates degree, Medical Record Certification (i.e. AHIMA), or equivalent\n* 3+ years of relevant work experience in healthcare industry with health information and/or direct medical record review and oversight\n* Comfort with multiple technology platforms, including but not limited to a variety of EMRs\n* Attention to detailĀ \n\n\n\n* PreferredĀ \n* Experience in pre-visit planningĀ \n* Experience with athenahealth\n* Experience with GSuite and Google applications\n\n\n\n\n\n$20 – $20 an hourThe base salary range for this full-time position is $20/hour + equity + benefits.Ā  Our salary ranges are determined by role, level, and location.Ā  The range displayed on each job posting reflects the minimum and maximum target for new hire salaries for the position.Ā  Within the range, individual pay is determined by work location and additional factors, including job-related skills, experience, and relevant education or training.Ā  Our talent team can share more about the specific salary range for your preferred location during the hiring process.\n\n#LI-Remote\n#LI-LB1\n\n\nFor Patient Facing Roles\nTo keep our patients, communities and each other safe, you’ll be required to comply with Accompany Health’s medical clearance requirements, including completing a TB screen and providing proof of immunity or vaccination for certain conditions.
